IMO this has a lot to do with women's place in the respective society. I know some may roll their eyes but when you look at societies where women are working careers they have far fewer children. Africa has a lot of problems but I think this is one of the most common factors among nations with very high birthrates.

And this is so because in rural areas, women plays a big role in the family economy while regular jobs are inexistant.
In urban area, a father of 6 will quickely appreciate his whife (already sold the other two) new salary to make ends meet at the end of the month.

Unfortunately I can't find any statistic or study about changes in behavior and natality in urban versus rural areas in third world countries.
